食事はおろか、水すら喉を通らなかった — why stack はおろか AND すら?

This sentence uses both intensifiers in one breath. Is はおろか～すら a fixed combo pattern, or are they just two separate emphasis devices that happen to be compatible? Could I swap すら for も and lose much?

はおろか is doing the 'not even close to A, let alone the extreme B' framing, and すら intensifies B specifically ('even B, of all things'). They stack because they're targeting different parts of the sentence — はおろか sets up the whole contrast, すら sharpens the shocking extreme. Swapping to も would still be grammatical but reads much flatter, almost like a normal list instead of a rhetorical escalation.
